Well Check Schedule

We recommend the following dates as guidelines for scheduling your child's well check appointments. Well checks allow providers to monitor the growth and development of your child in an extended visit while your child is healthy.

Well Check/Physical Schedule

• 5-10 days after discharge from the hospital
• 1mo of age
• 2mo
• 4mo
• 6mo
• 9mo
• 12mo
• 15mo
• 18mo
• 24mo
• 3yrs of age
• Yearly through age 18yrs
